If you are new to our practice, you have come to the right place! Below you will find the information forms we require all of our new patients to fill out. To expedite the check-in process once you arrive at our office, you can use Adobe Reader to download and print these forms and fill them out ahead of time!

Financing & Insurance
At Ascension Premier Dental, we want your experience at our office to be completely stress-free.
Because of this, we try to make paying for your dental care as simple as possible. We offer in-house financing and work with qualified dental plans. Of course, we also accept cash, personal checks, and major credit cards.
In-house Dental Savings Plan
The plan is available for individual patients and families. Our Dental Savings Plan can save you and your family thousands of dollars a year on dental treatments. Patients can pay a low annual fee and receive cleanings, exams, and X-rays.
We accept Proceed Finance, CareCredit & we offer In-House Financing.
CareCredit is a healthcare credit card that is designed for your health and wellness needs. For more information please log onto www.carecredit.com.
